Settlor: This is you, the person establishing the Trust. The Settlor will be required to sign the Trust before two witnesses and a notary.
Trustees(s): Generally, this is the Settlor, and any other adults whom you desire to have access to the NFA items held as property of the trust. All Trustees will be required to certify that they are legally capable of owning or possessing NFA items. Include anyone who will have any type access to the NFA items. You may have as many Trustees as you wish. If you need more space, please add the information at the bottom of this form in the additional information box.
Beneficiary / Beneficiaries: These people will not have access to the trust property until you elect to release the items, become incapacitated or die. It is common for Settlors to name their children as Beneficiaries, or other persons that they wish to ultimately take possession of the NFA items being held in the Trust. You may have as many Beneficiaries as you wish and they do not need to be adults. The Beneficiaries will not sign the Trust. If you need more space, please add the information at the bottom of this form in the additional information box.
Successor Trustee: This person will manage the trust if you become incapacitated or die. If a Beneficiary is a minor, and therefore incapable of possessing an NFA item, the Successor Trustee will determine if and when it is appropriate for the Beneficiary to receive the NFA items.
